Other Forms that You’ll Need
The forms listed below will be given to you during your initial visit to St. Elizabeth’s. If you’d like to fill them out in advance, download them below.
Teacher Recommendation Form (Kindergarten and First Graders): This is the form to use for students entering Kindergarten or First Grade. Fill out the top portion of this form and distribute it to your child’s preschool or kindergarten teacher, and have them mail it directly to St. Elizabeth’s School.
Teacher Recommendation Form (Second Grade and Up): This is the form to use for students entering Second Grade or higher. Fill out the top portion of this form and distribute it to your child’s current teacher, and have them mail it directly to St. Elizabeth’s School.
Student Transcript Release Request: Complete this form and send it directly to your child’s current school.
